If you have already filed your claim online, we will need additional documentation
to process your claim. Please see the checklist below for what may be required for
your claim type(s).
- Please make copies of all your documents and receipts for your personal records.
- Reference your claim number on every piece of documentation submitted.
- Once all necessary information has been obtained, we will process your claim within
seven business days.
Note: If you are filing a claim for medical reasons, you must send us a completed
Physician's Statement Form (PDF).
