Documentation Checklist

If you have already filed your claim online, we will need additional documentation to process your claim. Please see the checklist below for what may be required for your claim type(s).

  • Please make copies of all your documents and receipts for your personal records.
  • Reference your claim number on every piece of documentation submitted.
  • Once all necessary information has been obtained, we will process your claim within seven business days.

Note: If you are filing a claim for medical reasons, you must send us a Physician's Statement Form (PDF).

  1. General Documentation
    1. Receipts and itemized bills for all expenses.
    2. Original of any refunds or expense allowances received from your tour operator, travel agency, Common Carrier, resort, property management company, or other entity.
    3. Copy of Resort Invoice/Vacation Rental Contract or Confirmation(if applicable)

  2. Trip Cancellation/Interruption Claims
    1. Any appropriate documentation that officially explains the cause of your trip cancellation or interruption. Any explanation of diagnosis along with your original itemized bills, receipts, and proof of other insurance payments.
    2. Original unused tickets, copies of invoices, proof of payments, and other documents that substantiate the cost or occurrence of the trip cancellation or interruption.
    3. Documentation of refunds received from the travel supplier(s) and/or Common
      Carrier(s).
    4. Copy of the supplier's literature that describes penalties.
    5. A letter from the tour operator or an itemized bill from the travel agent stating the non-refundable amounts of the trip costs.

  3. Emergency Hospital and Medical Claims
  4. Any explanation of diagnosis(es) along with your original itemized bills, receipts, and proof of other insurance payment(s).

  5. Baggage Insurance Claims
    1. Original claim determination from the Common Carrier, if applicable.
    2. Original police report or other report of local authorities.
    3. Original receipts or credit card statement and list of stolen, lost or damaged items.
    4. Proof of loss providing amount of loss, date, time and cause of loss.

  7. Proof from the Common Carrier that personal baggage was delayed or misdirected for at least 24 hours.

  12. Trip Inconvenience Claims
    1. If admitted to a hospital, hospital billing records.
    2. If the victim of a felonious assault or involved in a car accident, an original police report.
    3. If passport is lost or stolen, a receipt from the consulate proving paid application for a new passport.
    4. If sporting equipment checked with a Common Carrier is delayed for more than 24 hours, supporting documentation from the Common Carrier.

  14. Original Common Carrier report that verifies the cause and duration of the delay.

    1. Original police report.
    2. Original rental car agreement.
    3. An itemized statement of repairs for the rental vehicle (unless the Company's adjuster has seen the car).

  18. Sports Traveler Cancellation/Interruption Claims

  19. Sporting Equipment Coverage Claims
    1. Original loss report and claim determination from the Common Carrier verifying that the sporting equipment was lost or damaged by the Common Carrier, if applicable.
    2. Original police report or other report from local authorities verifying that the sporting equipment was stolen, if applicable.
    3. Original receipts or credit card statement and list of stolen, damaged or lost sporting equipment.
    4. Repair estimate, if the sporting equipment is damaged.
    5. Proof of loss providing amount of loss, date, time and cause of loss.

  20. Sporting Equipment Rental Coverage Claims
    1. Proof from the Common Carrier that Your sporting equipment was damaged, lost or delayed for at least 12 hours, if applicable.
    2. Original police report or other report from local authorities verifying the theft of Your sporting equipment, if applicable.
    3. Original receipt for the sporting equipment rental costs.

  21. Vehicle Return Claims
    1. Copy of the vehicle registration or title.
    2. Original bill for total transport expenses.
    3. Copies of allowances received or expected from other insurers, or other parties.
    4. Signed Physician Form if your return was prevented due to an unexpected illness/injury.
    5. Original police report if the vehicle was stolen.
    6. Dollar amount being claimed.
