In order to process your claim, we need supporting documentation. You can find out what’s required for your claim type by clicking the "Descriptions of Required Documents" link below.

When you are ready to upload your documents(s), select your document type from the drop-down menu, and then click the "Select Document" button to choose your document(s). To add another document, simply repeat those steps.
When you are ready, proceed to "NEXT."
